Referrals
Information for patients referred to the hospital and health facilities referring patients to us.
If you have been referred to us
- Referral letter, where applicable
- National ID or other accepted identification
- Previous laboratory results, imaging reports and discharge summaries
- Health-cover details, where applicable
- Current medicines or a prescription
If you have been referred to a specialist clinic, check the Clinic Schedule for the current reporting day and time. For urgent or emergency conditions, do not wait for a scheduled clinic. Report directly to Accident & Emergency.
For referring health facilities
- Contact the hospital in advance when transferring a critically ill patient so the receiving team can prepare.
- Provide a clear referral letter with the patient's clinical history, findings, investigations, treatment given and reason for referral.
- Stabilise the patient before transfer according to the patient's condition and available resources.
- Ensure appropriate transfer arrangements and an escort for patients who require ongoing clinical support during transport.
- Send relevant records and investigation results with the patient where available.
We particularly welcome referrals for imaging including CT, surgery, obstetric emergencies, dialysis, newborn care and obstetric fistula assessment. These are services that previously required patients to travel out of Igembe region.
Onward referral from Nyambene
Some patients may require services that are not available at the hospital. Where onward referral is clinically indicated, the treating team will guide the patient or family on the next facility and referral arrangements.
The patient will receive appropriate referral documentation and relevant clinical information to support continuity of care.
