Medical Social Work
Psychosocial support, socio-economic assessment and follow-up for patients and families.
The Medical Social Work Service supports patients and families experiencing social, emotional and financial challenges related to illness and healthcare. The service works alongside clinical teams to address barriers to care, strengthen support systems and promote continuity of care.
The service is free of charge and confidential. Patients may be referred by any department of the hospital or seek support directly.
The service provides:
- Psychosocial support for patients and families.
- Socio-economic assessment where financial circumstances may affect access to care
- Counselling and support related to diagnosis, treatment adherence, disability and long-term care
- Follow-up and support for patients who miss scheduled appointments
- Linkage to community health promoters, social services and appropriate support programmes
- Assistance with SHA and health cover enquiries
- Safeguarding and support for vulnerable patient.
